How we estimate it
A phone camera has to resolve every module from where people stand. We size each module at no less than the scanning distance divided by 400, and never below 0.4 mm, which is about the smallest module a phone camera can reliably resolve. The whole code is never sized below 2 × 2 cm.
Then we multiply by the modules across the code, including the four-module quiet zone on each side. That is why a denser code — a longer payload or a higher error-correction level — needs more room at the same distance.
